Provider Attestation

CMS has recently provided further clarification on the information required from referring practices to enroll patients in GUIDE.

For each patient we are required to confirm, in addition to a dementia diagnosis ascertained from the ICD-10 code, that:

‘the patient meets the National Institute on Aging-Alzheimer’s Association diagnostic guidelines for dementia and/or the DSM-5 diagnostic guidelines for major neurocognitive disorder’

To ensure we maintain compliance with this updated guidance, and to further streamline the referral process, we are moving away from dropbox to a new secure portal.

The portal supports the two use cases

1. Individual referrals - We will continue to perform enrollment calls with any patient, irrespective whether we have a (complete) referral on file or not. For any patient that we dont have on file, we will create a ticket in the portal, that includes all the information we were able to get from the patient, as well as any missing data we need from the practice.
