Getting Started with GUIDE

Guiding an Improved Dementia Experience (GUIDE) is a CMS funded innovation program to support dementia patients and their unpaid caregivers.
To enroll patients and families in the program a practice must be approved by CMS and Craniometrix as an approved provider can help you to enroll your patients. Craniometrix provides Care Navigation services, access to respite care for qualifying families and caregiver education and support. These services are provided 24/7 in line with CMS requirements for the program. Your practice maintains responsibility for medical care of the patient.
Existing Eligible Patients
Your practice will have patients who are already eligible for GUIDE. Our team will work with you to understand, based on CMS criteria, the number of eligible patients.
📄 Patient Eligibility for GUIDE
Once we have this list each patient will require a FAST score, which can be easily derived from an existing MoCA or MMSE score. Patients meeting the eligibility criteria including a FAST of 4 or more can then be enrolled. We'll work with you collect the necessary data from your EHR, notify these patients and our team will complete intake assessments via phone or video.
Growing your GUIDE Patient List
Once eligible patients are addressed, our customer success team will work with you to find other patients. This may include:
Reviews of potentially eligible patients based on reviewing coding practices.
Developing processes for identifying and referring new diagnoses of dementia.
Marketing your participation in GUIDE to referral to referral partners and prospective patients.
